Belinda is the Assistant Data Coordinator for the RSCICDV. She is responsible for obtaining patient consents, collecting and reporting Form I data and also collects the interview data (FIM, CHART, Life satisfaction and Health Status) for the follow-up data collection. Belinda ensures that all patient admissions, transfers and discharges to the RSCICDV are recorded and that the appropriate personnel are notified of this information.
